We performed penetrating keratoplasty in the left eye of a 70-year-old female. The affected eye showed an elevated vascularized lesion of 5mm x 6 mm in size in the central cornea. Light microscopy showed irregularly arranged corneal epithelium and high basal cells. Bowman's membrane was irregular and partly interrupted. The corneal stroma showed irregular heterogenous collagen fibers and invasion by inflammatory cells and blood vessels. Electron microscopy showed nuclei with multiple large nucleoli and necrotic cells in the corneal epithelium with blood vessels in the subendothelium. Fibrocytes in the stroma lacked the cytoplasmic membrane. Above findings simulated those of filamentary keratitis. Inflammation near the Bowman's menbrane may have induced interruption, followed by epithelial proliferation leading to epithelial elevation and corneal opacity.
