急性前部ぶどう膜炎で発症した原田病の2例

Acute anterior uveitis as the initial manifestation in two cases of Vogt-Koyanagi-Harada disease

　59歳と53歳の男性が，線維素析出を伴う急性前部ぶどう膜炎を初発症状とする原田病を発症した。基礎疾患として，症例1に強直性脊椎炎，症例2に尋常性乾癬がみられた。両例とも副腎皮質ステロイド剤の大量投与で良好な最終視力が得られたが，症例1では原田病の診断が遅れ，遷延化した。急性前部ぶどう膜炎に関係する基礎疾患を持つ患者に発症する原田病は，通常とは異なる経過をとる場合があることを示している。

Two males, aged 59 and 53 years each, developed acute anterior uveitis with fibrous exudation as the initial manifestation of Vogt-Koyanagi-Harada (VKH) disease. Case 1 had ankylosing spondylitis and case 2 had psoriasis vulgaris. Megadose systemic corticosteroid resulted in good visual outcome. Case 1 ran a protracted course due to the delayed diagnosis as VKH disease. These cases illustrate that VKH disease may run an atypical clinical course when associated with some underlying diseases.
