42歳男性が3か月前からの両眼視力低下で受診した。28年前からIgA腎症があった。矯正視力は右0.5,左0.9であった。両眼に角膜後面沈着物を伴わない+2の前房混濁があった。隅角と蛍光眼底造影所見は正常であった。10か月間のステロイド薬点眼で前部ぶどう膜炎は寛解し,右0.7,左1.Oの視力を得た。間質性腎炎に伴うぶどう膜炎は多いが,本症例はIgA腎症でもぶどう膜炎が起こりうることを示している。
A 42-year-old man presented with decreased visual acuity in both eyes since 3 months before. He had been diagnosed with IgA nephropathy 28 years before. His visual acuity was 0.5 right and 0.9 left. Both eyes showed similar findings comprising no keratic precipitates and +2 cells in the aqueous. Gonioscopic and fluorescein angiographic findings were normal. After 10 months of topical corticosteroid, the anterior uveitis subsided with the visual acuity of 0.7 right and 1.0 left. There are several reports on uveitis associated with tubulo-interstitial nephritis. The present case illustrates that nongranulomatous anterior uveitis may occur associated with IgA nephropathy.
