　症例は71歳男性。全身倦怠感，食欲不振，頭痛で発症し，2カ月後に複視が出現した。左眼に第II，III，IV，VI脳神経障害があり，頭部MRIで両側海綿静脈洞から斜台にかけての腫瘤を認めた。頸部リンパ節腫大と右腹部腫瘤もあり，腹部腫瘤の針生検から悪性リンパ腫と診断した。悪性リンパ腫の海綿静脈洞への転移の頻度は比較的低いが鑑別する必要がある。

A 71-year-old man developed general fatigue, appe-tite loss, and headache. Two months later, he noticed diplopia. Examination demonstrated reduced visual acuity and complete ophthalmoplegia of the left eye. Brain MRI disclosed a mass that extended from bilat-eral cavernous sinus to the clivus. There were left cer-vical lymphadenopathy and a right abdominal mass. A needle biopsy of the abdomonal mass revealed non-Hodgkin lymphoma. Although malignant lymphoma at the cavernous sinus is not common, it should be an important consideration in the differential diagnosis of mass at the cavernous sinus.
