巨大裂孔を伴つた網膜剥離眼の治療成績

Results in Treatment of Retinal Detachment with Giant Breaks

緒言

　自発網膜剥離に対する手術術式の進歩は，本疾患全般の復位成績の向上に大きく寄与したが，難治条件をもつ症例のそれはなおはるかに低い水準に低迷している。難治条件の一つに巨大な裂孔があるが，これは比較的若年者にみることが多く，とるべき術式に苦慮せざるをえないものもある。Schepensらは経線範囲が90°以上にわたるすべての裂孔を巨大裂孔と定義して，その多数例について治療の方法と成績を報告しているが，その復位成績はやはり十分に満足すべきものとは思われない。私どもも最近約10年間にとり扱つた網膜剥離治療例において，Schepensの定義による巨大裂孔をもつものを集計して17人17眼をえたので，その概要を報告する。

During the past 10 years and 3 months, 17 cases involving 17 eyes with giant retinal breaks of 90°or more around the circumference of the globe as defined by Schepens were exa-mined at the Department of Ophthalmology, Hiroshima Uuiversity Hospital.

Six of the 17 eyes had disinsertion and 11 had retinal tear. The posterior retinal flap of 10 of the 11 eycs with retinal tear was inverted, but there were no such cases among those with disi-nsertion.

Surgery for retinal detachment was performed on 11 eyes excluding the one eye which was inope-rable because of giant break extending around the entire circumference of the globe.
