緑内障眼に対する網膜剥離手術例

Surgery for rhegmatogenous detachment in glaucomatous eyes

　緑内障眼に裂孔原性網膜剥離を合併した13症例における手術前後の視野変化と影響を及ぼす因子について検討した。視力経過は通常の網膜剥離と比較して差はなかったが，8例（62％）において視野の狭窄がみられ，視力は回復しても視野が悪化した症例が多く見られた。ガスタンポナーデや長期間にわたる眼底上昇，複数回の手術などが視野悪化の因子として考えられた。網膜剥離発症前からの緑内障性視野変化が，Ⅲ期以降の症例においては，78％と高率に視野悪化が認められた。

We reviewed the surgical outcome for rhegmatogenous retinal detachment in 13 eyes under treatment for glaucoma. The course of visual acuity was comparable to ordinary retinal detachment in nonglaucomatous eyes. Glaucomatous visual field defects progressed further in 8 of 13 eyes, 62%, and in 7 of 9 eyes with previously advanced visual field defects. Intraocular gas tamponade, postsurgical elevated intraocular pressure and repeated surgeries seemed to have contributed to the progression of visual field loss.
