We reviewed 26 eyes of 20 patients following vitreous surgery for untreated proliferative diabetic retinopathy during 3 years through 1996. The postoperative best visual acuity improved by 2 lines or more in 22 eyes (84%). It improved in 17 of 18 eyes (94%) without macular detachment and in 5 of 8 eyes (63%) with macular detachment. Visual deterioration during the follow up was frequent, resulting in poor final visual outcome. It was mainly due to glaucoma, cataract or macular degeneration. The findings show that vitreous surgery is indicated for untreated advanced diabetic retinopathy but that due precaution is necessary for postoperative glaucoma and macular lesions.
