初診時眼科未治療の増殖糖尿病網膜症患者の術後視力予後

Visual outcome following surgery for untreated proliferative diabetic retinopathy

（P1-1-21）　初診時に眼科未治療の増殖糖尿病網膜症20例26眼の硝子体手術を過去3年間に行い，その成績を検討した。術後最高視力は22眼（84％）で2段階以上改善した。黄斑剥離がない18眼では17眼（94％）で改善し，黄斑剥離のある8眼では5眼（63％）で改善した。術後の経過中に視力の再低下が起こる症例が多く，最終視力は概して不良であった。その原因には，緑内障，白内障，黄斑変性が主であった。初診時に眼科未治療で網膜症が進行している症例にも硝子体手術が有効であるが，術後の緑内障と黄斑障害に配慮する必要があると結論される。

We reviewed 26 eyes of 20 patients following vitreous surgery for untreated proliferative diabetic retinopathy during 3 years through 1996. The postoperative best visual acuity improved by 2 lines or more in 22 eyes (84%). It improved in 17 of 18 eyes (94%) without macular detachment and in 5 of 8 eyes (63%) with macular detachment. Visual deterioration during the follow up was frequent, resulting in poor final visual outcome. It was mainly due to glaucoma, cataract or macular degeneration. The findings show that vitreous surgery is indicated for untreated advanced diabetic retinopathy but that due precaution is necessary for postoperative glaucoma and macular lesions.
