We reviewed the clinical course of traumatic macular hole in 6 eyes of 6 patients. All were males and were aged from 10 to 28 years, average 17 years. Posterior vitreous detachment was consistently absent during the follow-up for 3 months or longer. The macular hole closed spontaneously in 4 eyes after 2 weeks to 5 months, average 10 weeks. The remaining 2 eyes underwent vitreous surgery 1 to 2 months after injury and resulted in closure of the hole. All the 6 eyes showed improvement in visual acuity by 2 lines or more. Based on the present findings and past reports, it is concluded that traumatic macular hole may undergo spontaneous closure and that vitreous surgery is to be advised in cases which persist for 3 months at least.
