A 46-year-old female visited our clinic with decreased visual acuity in her left eye. She had untreated diabetes mellitus. Her corrected visual acuity was 1.2 right and 0.15 left. Both eyes showed preproliferative diabetic retinopathy. The left eye showed disc hyperemia with swelling and caecocentral scotoma. Visually evoked cortical potentials (VECP) showed decreased amplitude and prolonged latency. Her left visual acuity recovered to 0.6 4 weeks after initiation of treatment with insulin. Another 4 weeks later, VECP showed reduced amplitude in the right eye followed by disc swelling and reduced visual acuity to 0.4. Six months after her initial visit, the visual acuities and visual fields became normalized with normalization of VECP findings. This case illustrates that VECP may be useful in diabetic papillopathy before onset of decreased visual acuity.
