偽水晶体眼網膜剥離の臨床所見

Clinical features of pseudophakic retinal detachment

（P1-2-21）　後嚢の温存されている，超音波乳化吸引術後偽水晶体眼網膜剥離15例15眼について臨床所見を検討した。白内障手術から網膜剥離発症までの推定期間は平均700日で，1年以降の発症が半数を占めていた。術前裂孔検出率は87％で，上耳側の赤道部に最も多くみられた。格子状変性を伴わない裂孔が6割，伴うものが4割であった。嚢外摘出術後に比べ下方の裂孔が多く，超音波の使用など術式の違いがその原因ではないかと考えられた。初回復位率は80％，最終復位率は100％で，初回硝子体手術を行った3例で再剥離をきたした。再剥離の原因は残存硝子体による新裂孔であった。

We reviewed 15 eyes of pseudophakic retinal detachment. All the eyes had received phacoemulsification-aspiration surgery preserving the posterior capsule. Retinal detachment developed after and average of 700 days of cataract surgery. In 8 eyes, it developed more than one year after surgery. Retinal break was detected in 13 eyes (87% ) . It was associated with lattice degeneration in 40% and not in 60% of eyes. It was located in the superior temporal equator in 60% of eyes, which was less than in reported cases following extracapsular cataract extraction. Retinal reattachment was obtained in 80% of eyes after initial surgery and in 100% of eyes after multiple procedures. Retinal detachment recurred in 3 eyes after vitrectomy as the initial procedure. Initial failure was caused by new tear formation due to traction of the remaining vitreous gel.
