視神経乳頭からの新生血管を伴ったベーチェット病に対する硝子体手術

Vitreous surgery for Behçet's disease with disc neovascularization

　視神経乳頭からの新生血管を伴ったベーチェット病に対して硝子体手術を行った。対象は2例2眼で，いずれの症例も両眼の視神経乳頭から新生血管が出現し，内科的治療にもかかわらず，硝子体出血を繰り返した。片眼は急速に牽引性網膜剥離を起こして失明したので，僚眼に対して牽引性網膜剥離を防ぐ目的で硝子体手術を行った。術前に比べて術後視力は2例とも向上し，手術に伴う新たな硝子体出血や重篤なぶどう膜炎の再燃などは生じなかった。ベーチェット病における視神経乳頭からの新生血管は，牽弓性網膜剥離の原因となることが予想される。比較的早期の硝子体手術は，牽引性網膜剥離への進行防止に有効であることが考えられる。

We performed vitreous surgery on 2 eyes of 2 patients with Behçet disease. Both cases had disc neovascularization and recurrent vitreous hemorrhage in spite of medical treatments. One eye of both cases had become blind due to tractional retinal detachment. Vitreous surgery on the fellow eyes resulted in improved visual acuity without vitreous hemorrhage or recurrence of uveitis. The present cases illustrate that disc neovascularization in Behçet disease may result in tractional retinal detachment and that timely vitreous surgery may be beneficial in its prevention.
