全眼筋麻痺を伴った眼部帯状疱疹の1例

76歳男性が全眼筋麻痺を伴う右眼部帯状疱疹を発症した。全眼筋麻痺は眼部帯状疱疹発症3日後にみられた。頭部MRI検査にて，髄腔内右三叉神経がガドリニウムによる増強効果を示し，対側に比較して海綿静脈洞が拡大していた。全眼筋麻痺は海綿静脈洞における三叉神経からの炎症の波及が原因と考えた。
A 76-year-old male presented with herpes zoster ophthalmicus in the right eye. Total ophthalmoplegia in the same eye developed three days later. Magnetic resonance imaging showed gadlinium enhancement in the right trigeminal nerve and dilated right cabvernous sinus. The total ophthalmoplegia appeared to have been induced by direct extension of inflammation in the cavernous sinus to the ipsilateral trigeminal nerve.
