アイゼンメンゲル症候群に発症した両眼性網膜中心静脈閉塞症の1例

A case of bilateral central retinal vein occlusion with Eisenmenger syndrome

　44歳女性が左眼視力低下で受診した。10年前に先天性心室中隔欠損が発見されたが放置していた。矯正視力は右1.2，左0.1であり，両眼眼底に網膜中心静脈閉塞症が発症していた。血液の凝固系と線溶系に異常はなく，赤血球数が790万／μlに増加していた。労作時に高血圧になる血圧変動があった。内科で心室中隔欠損症によるアイゼンメンゲル症候群および二次性多血症と診断された。両眼の網膜中心静脈閉塞症の原因として，血圧変動に伴う血流異常，多血症による血液粘稠度の上昇などのために，静脈系のうつ滞と血栓形成が起こりやすいことが推定された。

A 44-year-old female presented with blurring in her left eye as chief complaint. She had been diagnosed as ventricular septal defect 10 years before without follow-up treatment. Her corrected visual acuity was 1.2 right and 0.1 left. Funduscopy showed central retinal vein occlusion in both eyes. Laboratory studies showed normal coagulation system and erhthrocytemia at 7.9 × 106/μl. Systemic hypertension was induced when put under workload. She was diagnosed as Eisenmenger syndrome with secondary polycythemia. These featured appeared to have been the underlying cause for central retinal vein occlusion through venous stasis and thrombus formation.
