サルコイド性ぶどう膜炎でのツベルクリン反応と血清アンジオテンシン変換酵素活性測定の診断的意義

Diagnostic value of tuberculin skin test and serum angiotensin converting enzyme assay in sarcoid uveitis

　サルコイド性ぶどう膜炎を診断する上でのツベルクリン反応と血清アンジオテンシン変換酵素（angiotensin converting enzyme：ACE）活性測定の意義を，組織診断群のサルコイド性ぶどう膜炎56例についてretrospectiveに検討した。40例（71％）でツベルクリン反応が陰性で，38例（68％）で血清ACE活性が上昇していた。26例（46％）はツベルクリン反応陰性で血清ACE活性が高値であり，52例（93％）はツベルクリン反応陰性か血清ACE活性が高値であった。ぶどう膜炎の活動性はツベルクリン反応が陰性であることと関連していた。ツベルクリン反応と血清ACE活性測定の組合わせは，サルコイド性ぶどう膜炎の診断に有用と考えられる。

We reviewed histopathologically diagnosed 56 cases of sarcoid uveitis. Tuberculin skin test was negative in 40 cases (71% ) . Serum angiotensin converting enzyme (ACE) was elevated in 38 cases (68%) . Tuberculin test was negative and serum ACE was elevated in 26 cases (46%) . Either tuberculin test was negative or serum ACE was elevated in 52 cases (93%) . The activity of uveitis was positively correlated with negative tuberculin test. The findings show the usefulness of tuberculin test and serum ACE value in the diagnosis of sarcoid uveitis.
