We examined 10 eyes of hard exudates which appeared after vitrectomy for diabetic macular edema. Optical coherence tomography (OCT) showed the exudates as high-reflective patches. In 5 eyes, the inner portion of hard exudates was in apposition with the inner retinal surface. The corrected visual acuity averaged 0.5 in these 5 eyes. In the other 5 eyes, hard exudates were also present in the subretinal space. In OCT images, they formed a high-reflective plaque together with the retinal pigment epithelium (RPE) . The visual acuity in these 5 eyes averaged 0.08. The 5 eyes had serous retinal detachment in the macula either before or after vitrectomy. Subretinal hard exudates bridged the retinal and PRE in 2 eyes. The findings show that hard exudates, which appear after resolution of diabetic macular edema, may be located in the sensory retina and/or in the subretinal space. Eyes with subretinal exudate often result in poor visual outcome.
