円錐角膜に対するディスポーザブルソフトコンタクトレンズとハードコンタクトレンズの組合せ処方—角膜形状に及ぼす影響について

Effect of piggyback system with disposable soft contact lens on the corneal shape in keratoconus

　円錐角膜4例6眼に，ディスポーザブルソフトコンタクトレンズ（以下DSCL）とハードコンタクトレンズ（以下HCL）の組合せ処方（piggyback lens system）を行った。そして，処方時，処方1年前および1年後の角膜形状をコニコイド曲線X²＋（Q＋1） Y²−2RY＝0に近似し，円錐角膜形状異常程度を表すlndex K＝−（Q／R）×10³で円錐角膜形状の変化を評価した。その結果，1年間のIndex Kの変化量はpiggyback lens sys-temで−138.8±122.0,HGL単独装用で−64.2±88.6となり，DSCLを用いたpiggyback lenssystemの角膜形状改善効果は，少なくともHCL単独装用に匹敵するものと思われた。

We treated 6 eyes with keratoconus using a piggyback system with disposable soft contact lens. The corneal shape after 1 year of wearing was compared with that at the time of prescription and one year before when a conventional hard contact lens was in use. The corneal shape was expressed by an index K, or-(Q/R)×10³, which was obtained by approximating the corneal shape to a conicoid curve: X²+(Q+1) Y²-2RY=0. The index K during the period of hard contact wear was-64.2±88.6 and that during piggyback lens system wear was-138±122.0. The results indicate that the piggyback lens system is comparable to hard contact lens alone regarding the beneficial effect on the corneal shape in keratoconus.
