山口大学眼科における角膜移植手術成績

Surgical outcome of penetrating keratoplasty in our institution

（25D-8）　1996年3月までの30か月間に山口大学眼科および関連施設で施行した全層角膜移植術のうち，術後2か月以上当科で経過観察できた48例51眼につき，術前および術後経過を検討した。移植に至った原因疾患は水疱性角膜症22眼，角膜白斑11眼，円錐角膜10眼，角膜穿孔4眼，格子状角膜変性症3眼，その他1眼であった。全体の透明治癒率は78％であった。疾患別透明治癒率では角膜白斑および円錐角膜では極めて良好であったが，水疱性角膜症では59％とやや不良であった。透明治癒が得られなかった原因としては拒絶反応によるものが最も多かった。術後1年での内皮細胞減少率は28％であった。特に水疱性角膜症では42％の減少率であった。

We evaluated 51 eyes of 48 patients who underwent penetrating keratoplasty by us and who were followed up for 2 months or more. The causative lesions comprised bullous keratopathy 22 eyes, corneal leukoma 11 eyes, keratoconus 10 eyes, corneal perforation 4 eyes, lattice corneal dystrophy 3 eyes and others. The overall incidence of postoperative corneal transparency was 78%. Corneal leukoma and keratoconus showed very favorable results. Surgery was judged as success in 59% of eyes with bullous keratopathy. Graft rejection was the most frequent cause of opacity after surgery. Average rate of loss of corneal endothelial cells was 28% one year after surgery and was 42% in eyes with bullous keratopathy.
