膨隆白内障に対する前囊二重切開法

Double circular curvilinear capsulorhexis for intumescent cataract

膨隆白内障に対するヒーロンV^TM下での赤道部皮質吸引を併用した前囊二重切開法を報告する。前房中にヒーロンV^TMを充填して前囊を平坦にし，0.1％トリパンブルー溶液で前囊染色を行い，角膜サイドポートからチストトームで2mm程度の小さな円形前囊切開（CCC）を行った。角膜サイドポートから皮質吸引針を挿入し，そのCCCを通して赤道部付近の液状の水晶体皮質を吸引した。水晶体囊が膨隆しなくなった状態でCCCを拡大したのちに，超音波水晶体乳化吸引と眼内レンズ挿入を行った。現在までにこの方法で15例の手術を行い，1例でCCCを通して皮質吸引する操作で前囊に強く張力をかけたため亀裂を生じた。本法は，この点に注意すれば成功率の高い手術方法となる可能性がある。

We developed a surgical technique for intumescent cataract utilizing double circular curvilinear capsulorhexis. It proceeds as follows. The lens capsule is stained with 0.1％ trypan blue after injection of hyaluronic acid（HealonV^TM）into the anterior chamber. Circular curvilinear capsulorhexis of 2 mm in diameter is performed by a cystotome through a corneal sideport. Lens cortex is aspirated by suction needle through the opening in the lens capsule. The opening in the capsule is enlarged after removal of milky cortex,to be followed by phacoemulsification and intraocular lens insertion. We used this technique in 15 eyes with intumescent cataract. Anterior capsule tear developed in one eye during aspiration of the cortex due to tension in the capsule. There was otherwise no intra- or postoperative complication. Our method promises to be useful in surgery for intumescent cataract.
