We performed scleral buckling surgery for retinal detachment in 1,101 eyes during the past 4 years. Anterior segment ischemia developed postoperatively in 11 eyes. These eyes were characterized by multiple retinal tears with lattice degeneration located in the superior and inferior equator. This feature necessitated the use of long and wide silicone implants and multiple surgeries. Anterior segment ischemia manifested as temporary corneal edema, narrow chamber angle, ocular hypertension with shallow anterior chamber, persistent inflammation, iris atrophy with posterior synechia, and cataract formation. Final visual acuity was poor in spite of anatomical reattachment. Four cases were treated by intravenous corticosteroid with equivocal effects.
