Anterior segment ischemia after scleral buckling for retinal detachment

　1991年8月から1994年12月までに，裂孔原性網膜剥離に対して強膜バックル手術を施行した1,101眼のうち，11眼（1.0％）に前眼部虚血（ASI）が発生した。術前の特徴は赤道部に位置する広範囲の網膜格子状変性に多発性の馬蹄型裂孔を有するものが多かった。手術は赤道部に広範囲のシリコン埋没術を行ったものや，複数回の手術を施行したものに多かった。ASIの特徴として，可逆性角膜浮腫，一過性浅前房による隅角閉塞と眼圧上昇，遷延性炎症，虹彩萎縮と癒着，白内障の進行がみられた。網膜は復位しているにもかかわらず術後視力の低下するものが多かった。

We performed scleral buckling surgery for retinal detachment in 1,101 eyes during the past 4 years. Anterior segment ischemia developed postoperatively in 11 eyes. These eyes were characterized by multiple retinal tears with lattice degeneration located in the superior and inferior equator. This feature necessitated the use of long and wide silicone implants and multiple surgeries. Anterior segment ischemia manifested as temporary corneal edema, narrow chamber angle, ocular hypertension with shallow anterior chamber, persistent inflammation, iris atrophy with posterior synechia, and cataract formation. Final visual acuity was poor in spite of anatomical reattachment. Four cases were treated by intravenous corticosteroid with equivocal effects.
