難治性と思われた糖尿病網膜症に対する硝子体手術例の検討

Outcome of vitreous surgery for severe diabetic retinopathy

（展示225）　難治性と思われた増殖糖尿病網膜症40眼に硝子体手術を行い，視力予後に関係する要因を検討した。術後視力が向上あるいは不変であった症例は24眼（60％），悪化した症例は16眼（40％）であった。術前の網膜剥離の有無，光凝固の有無，術中網膜裂孔の有無，術中大出血の有無は，視力予後には影響しなかった。術翌日からの硝子体出血は有意に視力予後に悪影響を与えた（p＜0.05）。術終了時の大量の凝血塊残存も視力予後を悪化させる傾向にあった（p＜0.1）。重症糖尿病網膜症の手術では，眼内に大量の凝血塊を残さないことと，術中の止血を十分に行うことが重要であると結論される。

We reviewed 40 eyes of advanced proliferative retinopathy treated by pars plana vitrectomy. After surgery, the visual acuity improved or remained stable in 24 eyes (60%) and deteriorated in 16 eyes (40%). Following factors did not influence the final visual outcome : presence of retinal detachment, prior photocoagulation, intraoperative retinal tear, and intraoperative massive hemorrhage. Following two factors adversely affected the visual outcome : vitreous hemorrhage on day 2 of surgery (p＜0.05) and persistent blood clot at the end of surgery (p＜0.1).
