赤血球，血小板減少を伴った両眼性網膜中心静脈閉塞症の1例

A case of bilateral central retinal vein occlusion with erythrocytopenia and thrombocytopenia

　糖尿病と慢性C型肝炎が原因と思われる赤血球，血小板減少を合併した両眼性網膜中心静脈閉塞症の1例を経験した。患者は61歳男性で，左眼視力低下を主訴に受診した。眼底は糖尿病網膜症に合併した両眼性の網膜中心静脈切迫閉塞症を示し，全身検査では赤血球，血小板の減少がみられていた。

　網膜中心静脈閉塞症発生に関与する因子として，高血圧，糖尿病，動脈硬化などがあるが，これに加え血液学的因子による易出血性にも留意し，厳重に経過観察することが必要であると考えた。

A 61-year-old male presented with decreased vision in the left eye of 3 months' duration. He had suffered from chronic hepatitis C 8 years before. Diabetes mellitus had been detected 3 months before. Both eyes showed impending central retinal vein occlusion. Laboratory studies showed erythrocytopenia and thrombocytopenia. Patients who develop bilateral central retinal vein occlusion may have associated systemic hypertension, arteriosclerosis or diabetes mellitus. This case illustrates that hemorrhagic diathesis may serve as an underlying cause for central retinal vein occlusion.
