糖尿病網膜症に合併したインターフェロン網膜症

Interferon-associated retinopathy in eyes with diabetic retinopathy

　糖尿病網膜症を伴った慢性C型肝炎に対するインターフェロン（IFN）投与後にIFNにより眼底所見の悪化した症例を2例経験し，若干の考按を加えた。IFNによる出血は，乳頭周囲の表在性出血が特徴で糖尿病網膜症による深在性出血と区別ができ，また保存療法で消退傾向がみられた。また症例により糖尿病網膜症の悪化が主体のものとIFN網膜症が主体のものとに違いがみられたが，これは年齢，糖尿病罹患年数，IFNの種類・投与方法の関与が考えられた。発生機序はC型肝炎ウイルス，IFNどちらの関与も考えられた。

Two cases of diabetic retinopathy developed interferon-associated retinopathy during treatment for chronic hepatitis C. The hemorrhage in interferon retinopathy was superficially located and was different from deep hemorrhage characteristic for diabetic retinopathy. The hemorrhage spontaneously resolved by systemic medication. Diabetic retinopathy was the cardinal feature in one case and interferon retinopathy was prominent in the other. The difference appeared to be due to age, duration of diabetes, type and mode of administration of interferon. It appeared that the interferon retinopathy was expession of antigen-antibody or immunocomplex reaction due to hepatitis C. Spasm of retinal artery due to interferon will have played an accessory role.
