Paradoxic levator inhibitionの1例

A case of paradoxic levator inhibition

　3歳男児のparadoxic levator inhibitionの1例を経験した。生後から，左眼内転時に左眼の眼瞼下垂が生じるが，本症例では，上方視時には左眼が過剰上転するという異常連合運動を伴っていた。また全身的にはファロー四徴症を合併していた。患児は，指標固視時には眼瞼下垂を生じることは少なく，視力，両眼視の発達は良好であった。また成長につれて眼瞼下垂の頻度は減少しており今後も改善していくことが期待される。

　Paradoxic levator inhibitionの発生機序は不明であるが，胎生期に何らかの因子が働き，眼瞼運動や眼球運動を支配する神経の中枢性の異常によると椎察された。

A 3-year-old male infant presented with paradoxic levator inbibition since birth. He manifested ptosis of left eyelid during adduction associated with overshooting of the eye during supraduction. Fallot's syndrome was also present. Visual function and binocular vision were good due to minimal ptosis during fixation in the primary position. The ptosis showed spontaneous improvement during the ensuing 22 months of follw up. Unidentified factors during the prenatal period appeared to have affected central innervation for palpebral and ocular motility.
