眼窩神経鞘腫の画像診断

Magnetic resonance imaging of orbital schwannoma

　眼窩内神経鞘腫の3例について画像（特にMRI）と病理組織学的所見との比較を行い，その特徴について検討した。外部形状では全例が境界明瞭な円，楕円形を示し，被膜を有する組織形態を反映していた。内部陰影では，病理組織学的分類のAntoni A型に相当する領域は造影MRlにて著明な造影効果を示したのに対し，Antoni B型に相当する領域は造影効果をほとんど示さなかった。また組織学的に多結節型を示した1例は造影MRIにてモザイク状の陰影として描出された。今回のMRIにおける解析結果と病理組織学的所見との比較から，MRIは腫瘍の外部形状，内部陰影をかなり微細なレベルまで画像上に描出が可能であることが確認された。

We reviewed MRI and histopathological features of 3 cases with orbital schwannoma. MRI showed round and well-delineated masses in all cases, indicating the characteristics of encapsulated tumor. Highly ordered cellular Antoni-A type was strongly enhanced on postcontrast T1W1. Loose myxoid Antoni-B type was poorly enhanced. Multinodular pattern in histopathological features appeared as mosaic on MRI. The findings show close correlation between MRI and pathological features of the tumor.
