両眼に同一クローンのBリンパ球の増生を確認した結膜悪性リンパ腫

Genotypic identity of B lymphocyte clone in bilateral conjunctival malignant lymphomas

　両眼性の結膜リンパ性腫瘍の2症例を経験した。病理組織学的検索では1例は異型に乏しいリンパ球の単調な浸潤からなり慢性結膜炎との鑑別が問題となった。1例はmucosa associated lymphoidtissue （MALT）リンパ腫の形態を示した。この組織の免疫グロブリン遺伝子の検索を行ったところ2症例ともに免疫グロブリン重鎖遺伝子の再構成が認められ，2症例ともにB細胞悪性リンパ腫と診断された。さらにこの2症例の腫瘍は左右ともに遺伝子再構成バンドが同一であったことから，左右ともに同—クローンのBリンパ球を起源とする両側性の悪性リンパ腫と考え，その原発部位について考察した．

We performed genotypic analysis in two cases of bilateral conjunctival lymphoid neoplasms. Histological examinations showed chronic inflammatory changes with monotonous lymphocyte infiltrations in one case and features of mucosa associated lymphoid tissue lymphomas in the other. They exhibited immunoglobulin heavy chain gene rearrangement, indicating monoclonal B lymphocyte proliferation. In either case, the pattern of rearrangement bands with southern blot hybridization was identical in both eyes. This finding indicated that the conjunctival lymphomas in both eyes were derived from identical B lymphocyte clone.
