眼部脂腺癌9例の検討

Review of nine cases of sebaceous carcinoma of ocular adnexa

　眼部脂腺癌9例（眼瞼部8例，涙丘部1例）の治療法と転帰について検討した。最大径5mm以下の眼瞼部脂腺癌3例では2mmの健常部をつけて，また最大径10mm以上の5例では7〜10mmの健常部をつけて切除を行った。後者の群の1例に転移，他の1例には転移による死亡が認められた。眼瞼部脂腺癌は外見上，霰粒腫と鑑別が困難な例があるので，高齢者で霰粒腫様症状を認めたときには，積極的に眼瞼部脂腺癌を疑い，まだ腫瘍の小さい早期に悪性腫瘍であることを考慮した治療を開始すべきであると推論された。

We reviewed 9 cases of sebaceous carcinoma of ocular adnexa regarding the therapeutic modality and outcome. The tumor originated from the eyelid in 8 cases and from the lacrimal caruncle in 1. In 3 cases of sebaceous carcinoma of eyelid measuring 5 mm or less in size, the tumor was dissected containing 2 mm of healthy skin around the tumor. In 5 cases measuring 10 mm or more, the tumor was dissected containing 7 to 10mm of healthy skin. Metastasis or tumor-related death resulted in 2 in the latter group. Malignancy is to be suspected in elder persons manifesting chalazion-like lesions, as early surgical extirpation is advocated while the lesion is still small.
