脈絡膜悪性黒色腫のインドシアニングリーン螢光眼底造影所見

Indocyanine green angiographic findings of choroidal malignant melanoma

　脈絡膜悪性黒色腫の1例を経験した。症例は，42歳男性。右眼黄斑部耳側に漿液性網膜剥離を伴った有茎性の充実性腫瘤がみられた。大きさは，15×12×10mmであった。

　フルオレセイン螢光眼底造影（FA）とインドシアニングリーン赤外螢光眼底造影（IA）を施行し，比較検討した。FAで，明らかな異常血管を認めなかった部位に，IAでは脈絡膜異常血管が観察でき，その異常血管からびまん性の色素漏出もみられた。

　IAを用いることにより，眼内の隆起病変に対して，FAでは検出できない脈絡膜異常血管が観察可能となり，腫瘍の発生および悪性度や伸展度の判定に有用ではないかと思われる。

A 42-year-old male presented with malignant melanoma of the choroid located temporal to the macula in his right eye. The tumor measured 15mm×12mm in size and 10mm in height. Serous retinal detachment was present in the inferior periphery. Indocyanine green (ICG) angiography showed abnormal vessels in the tumor and dye leakage from these vessels. These vessels failed to be detected by conventional fluorescein angiography. ICG angiography appeared to be of value in differential diagnosis of elevated suberetinal lesions regarding the nature of the tumor, malignancy and its extent.
