A 19-year-old female presented with bilateral conjunctival hyperemia and ocular pain of 4 weeks' duration. Bilateral exophthalmos developed 4 weeks later. Orbital myositis involving all extraocular muscles was suspected on computed tomographic findings. Systemic prednisolone at the initial daily dosis of 120mg induced rapid improvement in clinical findings. They recurred when the dosage was reduced to 20 mg daily 30 days later. Biopsy of right medial rectus showed nonspecific infiltration of inflammatory cells. Temporary increase of prednisolone to the initial dosis, to be tapered in 172 days, resulted in permanent disappearance of clinical signs 40 days later. The present case illustrates that care is necessary in tapering off systemic corticosteroid for protracted orbital myositis with marked thickening of extraocular muscles.
