両眼性外眼筋炎の1例

A case of bilateral orbital myositis

　19歳の女性が両眼の充血と眼痛を主訴として受診した。その後，両眼眼球突出，複視をきたし，眼科的一般検査，CTスキャンにより両眼の外眼筋炎と診断された。ステロイド治療でいったんは改善したが，減量とともに再発し，さらに高度の外眼筋肥厚がみられたため，筋生検を行ったところ組織学的には非特異的な炎症所見であった。再度ステロイド治療を試み比較的徐々に減量したところ治癒した。

　本症例は発症約2か月後に治療が開始され，しかも両眼性に4直筋すべてが高度に肥厚した重度の外眼筋炎であった。このような症例に対してはステロイドの減量は慎重に行う必要があると思われた。

A 19-year-old female presented with bilateral conjunctival hyperemia and ocular pain of 4 weeks' duration. Bilateral exophthalmos developed 4 weeks later. Orbital myositis involving all extraocular muscles was suspected on computed tomographic findings. Systemic prednisolone at the initial daily dosis of 120mg induced rapid improvement in clinical findings. They recurred when the dosage was reduced to 20 mg daily 30 days later. Biopsy of right medial rectus showed nonspecific infiltration of inflammatory cells. Temporary increase of prednisolone to the initial dosis, to be tapered in 172 days, resulted in permanent disappearance of clinical signs 40 days later. The present case illustrates that care is necessary in tapering off systemic corticosteroid for protracted orbital myositis with marked thickening of extraocular muscles.
