Abstract Background:Within the scope of idiopathic orbital inflammation, the incidence of concomitant idiopathic extraocular myositis and dacryoadenitis is rare. In this report, we present a case of idiopathic extraocular myositis originating from dacryoadenitis.

Case:A 76-year-old man was referred to the Musashikosugi Hospital due to right eye pain, congestion, eyelid swelling, and decreased visual acuity. A diagnosis of idiopathic lacrimal gland inflammation was confirmed through lacrimal gland biopsy. Subsequent administration of steroid therapy initially improved his symptoms. Six months later, the patient developed extraocular myositis, which again responded positively to steroid therapy.

Conclusion:We encountered a case in which external ophthalmoplegia developed subsequent to dacryoadenitis. Idiopathic orbital inflammation may migrate to different sites, emphasizing the necessity of long-term clinical observation.
