AIDS患者に発症したcytomegalovirus視神経炎

Cytomegalovirus optic neuritis in a patint of acquired immunodeficiency syndrome

　後天性免疫不全症候群に合併したcytomegalovirus （CMV）視神経炎を報告した。51歳男性で入院加療中であったが，CMV抗体の高値が持続するので眼底を定期的に観察していた。7か月後に左眼視力が低下し，眼底検査で左眼視神経乳頭の腫脹と周囲網膜の出血，滲出斑を認めた。CMVによる視神経炎を疑いガンシクロビルを投与したところ，病変は2か月で鎮静化した。副作用のため投与量を減量したところ，左眼網膜に顆粒状の小滲出斑が出現し，3か月後にはCMV網膜炎の眼底所見を呈した。視神経炎で発症したCMV網膜炎は欧米では報告がある。本症例の特徴は一致していた。

A 51-year-old male with acquired immunodeficiency syndrome was referred to us due to elevated antibody titer for cytomegalovirus. Sudden decrease in vision developed in the left eye 7 months later. Clinical studies led to the diagnosis of acute optic neuritis. In spite of systemic ganciclovir, progressive retinitis appeared resulting in loss of vision due to macular involvement. The present case was consistent with reported cytomegalovirus optic neuritis.
