A 28-year-old male presented with blurred vision and hyperemia in the left eye of 2 weeks' duration. The affected eye showed granulomatous iritis and, funduscopically, disc hyperemia, dilated and tortuous retinal veins, exudative focus temporal to the macula and patchy exudates in the periphery. The fundus findings progressed rapidly after systemic prednisolone for 1 week. The condition started to subside after systemic acyclovir, interferon beta and aspirin. Rhegmatogenous retinal detachment necessitated vitreous surgery with scleral encircling procedure 5 weeks after onset of symptoms. Anterior chamber fluid and vitreous obtained during surgery showed elevated titers for varicella zoster virus, VZV. DNA of VZV was proved in the vitreous aspirate by polymerase chain reaction. The present case was unique in manifest-ing retinal exudates without fluorescein leakage from retinal vessels. The findings seemed to indicate that pathological process in the retinal pigment epithelium or the choroid preceded retinal vascular lesions.
