間質性腎炎とぶどう膜炎を合併した2例

Two cases of tubulointerstitial nephritis with uveitis syndrome

　間質性腎炎にぶどう膜炎を合併した2症例を経験した。症例1は10歳の女児で，両眼の毛様充血，前房中の細胞微塵を認めた。右眼は硝子体混濁，視神経乳頭の発赤腫脹，網膜血管炎があり汎ぶどう膜炎であった。同時に，腎機能障害を認め，腎生検にて間質性腎炎と診断された。症例2は62歳の女性で，左眼前房にフィブリン塊の析出を伴う前部ぶどう膜炎を認めた。さらに，尿中β₂ミクログロブリン，N—アセチルβ—D—グルコサシニダーゼの高値を認め間質性腎炎が強く示唆された。腎機能障害は症例1はステロイド療法により，症例2は対症療法にて軽快した。ぶどう膜炎は2症例とも再燃はみられたがステロイド療法にて改善傾向を示した。眼所見と腎機能，血清補体価およびIgG値などの変動が一致していた。本2症例の臨床経過より，腎尿細管障害とぶどう膜炎との強い関連が示唆された。

We observed two cases of tubulointerstitial nephritis with uveitis (TINU) syndrome. A 10-year-old girl with recently diagnosed renal dysfunction presented with bilateral iritis. The right eye showed vitreous opacity, swollen optic disc and retinal vasculitis. Renal biopsy showed interstitial nephri-tis. A 62-year-old woman showed anterior uveitis and fibrinous mass in the anterior chamber in theleft eye. General fatigue led to detection of elevated urine values of β₂-microglobulin and N-acetyl-β-D glucosaminidase, suggesting tubulointerstitial nephritis. Renal functions improved by systemic corticosteroid in the first case and by symptomatic treatments in the second. Relapse of uveitis was controlled by topical corticosteroid in both cases. The activity of uveitis was well correlated with renal dysfunction, serum levels of compliments and IgG.
