IgA腎症にぶどう膜炎を合併した1例

A case of uveitis associated with IgA nephropathy

　間質性腎障害を伴うIgA腎症にぶどう膜炎を合併した1例について検討した。両眼に微細な角膜後面沈着物，前房内にフレアおよび細胞，前部硝子体に炎症性細胞，両視神経乳頭の軽度発赤を認め，網膜静脈は一部拡張し，螢光眼底造影にては両視神経乳頭の過螢光を認めるなど，尿細管・間質性腎炎とぶどう膜炎症候群と同様の臨床症状であったが，新たな所見として片眼にDalen-Fuchs結節様の沈着物を認めた。IgA腎症は腎糸球体の炎症であるが，本症例のように間質性腎障害を伴うことも多い。本症例の発症機序として，腎糸球体と尿細管とぶどう膜に共通な抗原の存在の関与が推測された。

Uveitis developed in a male with IgA nephropathy with interstitial disorder. Both eyes showed fine keratic precipitates, flare and cells in the anterior chamber and inflammatory cells in the anterior vitreous. Funduscopy showed hyperemic disc and partial dilatation of retinal veins in both eyes. The clinical features simulated tubulo-interstitial nephritis and uveitis syndrome. One eye manifested Dalen-Fuchs nodules. While IgA nephropathy is basically a glomerular inflammation, many patients manifest interstitial disorders. An antigen common to glomerulus, renal tubule and uvea appeared to underlie in this disorder.
