白内障手術を施行した重篤な壊死性強膜炎

Cataract surgery in an eye with severe necrotizing scleritis

　重症の壊死性強膜炎にて約半周にわたる強膜ぶどう腫が認められた症例に対し，残存した健常な強膜から白内障手術を行った。

　症例は82歳の男性で，約2年前から近医にて壊死性強膜炎に対しステロイド点眼，内服治療を受け，約1年前から炎症は鎮静化していた。その後急速に進行した白内障に対して，今回，水晶体嚢外摘出術および眼内レンズ挿入術を行った。術後虹彩炎は中等度で強膜炎の再発はなく，矯正視力1.0となった。長期の経過観察が必要ではあるが，重症な壊死性強膜炎に対し白内障手術を行った稀な症例として報告した。

A 82-year-old man developed scleral staphyloma after severe necrotizing scleritis in the nasalsector in both eyes. He had been under topical and systemic corticosteroid therapy 2 years before. Thenecrotizing scleritis remained in cured state since 1 year. Due to progression of cataract, we performedextracapsular cataract extraction with intraocular lens implantation in the right eye. The incision wasplaced in the residual intact corneoscleral portion. The postsurgical course was uneventful with minimaliritis, no recurrence of scleritis and visual acuity of 1.0.
