脊髄小脳変性症と交代型skew deviation

Alternating skew deviation in spinocerebellar degeneration

　脊髄小脳変性症に交代型skew deviationを認めた10症例について検討した。脊髄小脳変性症による神経症状が出現してから本症候による複視が発現するまでの期間が，10症例のうち5症例で1年以内であったことから，交代型skew devia—tionが脊髄小脳変性症の初期症状のひとつとして捉えられると考えた。また，交代型skew deviationの責任病巣として従来は中脳水道近傍が重要視されているが，今回の10症例では随伴症候から小脳や下部脳幹がその責任病巣と考えられ，両者のいずれの病変でも起こり得ることが推察された。

We evaluated 10 cases of spinocerebellar degen-eration (SCD) manifesting alternating skew devia-tion. The series included 7 cases of late cortical cerebellar atrophy and 3 of olivo-ponto-cerebellar atrophy. Five patients noted diplopia due to alter-nating skew deviation within one year after neur-ological signs of SCD became manifest. It appeared that alternating skew deviation is an early sign of SCD. Neurological features and ocular movement pattern indicated the lesions to be located in the cerebellum and lower brainstem. Alternating skew deviation seemed to be due to lesions in the peria-quaduct area, cerebellum, or lower brainstem.
