後部硝子体剥離に伴い軽快した糖尿病黄斑浮腫の1例

Diabetic macular edema improved after posterior vitreous detachment

　筆者らは，糖尿病黄斑症の経過中，後極部での部分的な後部硝子体剥離が生じるとともに黄斑浮腫が急速に軽減した53歳男性の糖尿病網膜症症例を経験した。本症例はまもなく左眼に硝子体出血と牽引性網膜剥離を併発し，再度の視力低下をきたしたため，硝子体手術を行った。術中，肥厚した後部硝子体膜は中心窩に強固に癒着しており，これを切除し完全な後部硝子体剥離を作成した。術後は網膜症そのものも鎮静化し，現在まで黄斑浮腫の再発をみていない。この症例の経過から，硝子体手術による完全な後部硝子体剥離の作成は黄斑浮腫に対する積極的治療法となる可能性が示唆された。

A 53-year-old male with diabetes mellitus of 12 years' duration sought advice due to blurrin in his left eye. The left fundus showed tortuosity and dilatation of retinal veins, diffuse retinal edema and hard exudates in the macula. The eye was treated by panretinal photocoagulation. The visual acuity was 0.1 initially and deteriorated to 0.02 7 months later. The vision showed rapid improvement to 0.1 another 2 months later following spontaneous posterior vitreous detachment in the macular area. While vitrectomy became necessary later due to residual vitreoretinal adhesion and its complications, spontaneous or surgi-cally induced separation of posterior hyaloid membrane and the macula seemed to be benefical to diabetic macular edema.
