糖尿病網膜症に対する早期硝子体手術の術後経過

Clinical course following early vitrectomy for diabetic retinopathy

　早期硝子体手術を行った増殖糖尿病網膜症症例32眼の視力経過を検討した。手術適応から，硝子体出血など出血を伴った出血群と，黄斑牽引など出血を伴わない非出血群に分類すると，術後1年の矯正視力が0.5以上の症例の割合は，出血群70％，非出血群56％とともに良好であり，約半数の例では術前3か月と比較しても視力が向上していた。しかし非出血群では術後の視力回復の不良な例があり，これは手術施行の明確なきっかけがなく，適切な硝子体手術時期を逸したためと考えられた。したがって，非出血群ではより良好な視力保持の点から，さらに早期の手術が望ましいと考えられた。

We evaluated the outcome of early vitrectomy for diabetic retinopathy in 32 eyes. The series comprised 23 eyes with recent vitreous and/or preretinal hemorrhage and 9 eyes with fibrovascular prolifera-tion and/or macular distortion without hemorrhage. At one year after surgery, visual acuity of 0.5 or better was obtained in 70% of eyes in the former group and in 56% in the latter. About one-half of either group showed better postoperative vision than the value 3 months before surgery. Three eyes in the latter group, 33%, failed to show improved visual acuity, due to prolonged tractional damage to the macula with resultant irreversible visual deterioration. We advocate earlier vitrectomy for diabetic eyes with fibrovascular proliferation with macular distortion.
