成人発症黄斑色素上皮ジストロフィーと思われた1症例

A case of presumed adult-onset foveomacular pigment epithelial dystrophy

　53歳男性の両眼黄斑部に卵黄様病巣を認めた。病巣の大きさは，1／2〜1／3乳頭径でやや隆起し，螢光造影所見では，初期には病巣部に一致したブロック像を示し，後期にはわずかに過螢光を示した。EOG検査ではL／D比の低下を認めなかった。2年の経過観察で病巣部は顆粒状に変化してきており，1年後の螢光造影所見では環状に近い螢光像を示した。Scotopic・photopic ERGは正常であったが，局所ERGを行った結果，中心5°では低下が強く，その他でほぼ正常な値を示したため感覚網膜の障害も病巣部位に限局していることが示唆された。

A 53-year-old male presented with vitelliform macular lesions in both eyes. The lesions were slightly elevated and measured 1/2 to 1/3 disc diameters. Fluorescein angiography showed block-ed fluorescence during the early phase and slight staining towards the late phase. Both eyes showed normal light-peak/dark -trough ratio in electro-oculogram. Eighteen months later, the vitelliformlesions showed a granular appearance and ring-shaped hyperfluorescence in fluorescein angiography.Scotopic and photopic electroretinogram. Local ERG at the central angle of 5 degrees was sub-normal. The findings seemed to suggest that the damage of sensory retina was located in the foveal and parafoveal region.
