単焦点後房レンズ挿入眼における術後視力良好群の検討

Factors for good visual acuity after posterior chamber implantation of monofocal intraocular lens

　後房型単焦点眼内レンズ挿入術を施行した100眼において，術後の非追加矯正視力が遠方，近方視力とも0.7以上の視力良好群と，それ以外の視力不良群とに分け，年齢，他覚的屈折度，瞳孔径，瞳孔形状，虹彩後癒着の有無，対光反応について比較検討した。その結果，19眼が視力良好群で，視力不良群と比較すると他覚的屈折度は，より軽度の近視傾向を認め，平均年齢が若く，瞳孔径が小さく正円で，虹彩後癒着のない，対光反応の良好な症例が多く認められた。これらより，視力良好群における偽調節の機序としては，軽度の近視性乱視における散光圏効果とともに，近見時における良好な縮瞳反応が，焦点深度の増大に関与しているものと考えられた。

We reviewed 100 eyes which underwent posterior chamber implantation of monofocal intraocular lens. Nineteen eyes in the series had uncorrected visual acuity of 0.7 or better both for far and near. We evaluated the 19 and the other 81 eyes regard-ing age, postoperative state of refraction, pupil shape and diameter and the presence of posterior synechia. These 19 eyes were characterized by younger age, lesser grade of myopia, smaller and round pupil. The pseudoaccommodation in the 19 eyes was induced by low myopia of -0.5 diopter with minimum astigmatism as well as small round pupil.
