後房レンズ挿入眼での前房隅角の変化

The anterior chamber angle following posterior chamber lens implantation

　後房レンズ挿入眼の隅角変化，眼圧および房水流出率について術式，眼内レンズの固定部位別に検討した。

　術式や眼内レンズの固定部位により隅角部にPASやpigment dispersionが生じたが，発生部位，頻度および程度に差が認められた。眼圧および房水流出率は正常であるが，それぞれ74眼中4眼と5眼に悪化例が生じ，これらは隅角に広範囲のPASや程度の強いpigment dispersionを伴う症例にみられた。

　眼内レンズ挿入の際には眼組識への侵襲を少なくする配慮とともに術後の眼所見の緻密な観察を行い緑内障の発生に対処するべきである。

We evaluated the gonioscopic features after im-plantation of posterior chamber lens in 74 eyes. There were differences in the incidence of periph-eral anterior synechia (PAS) and pigment disper-sion depending on the type of surgery and loop fixation. PAS was more frequent along the 10 to 2 o'clock position. Pigment dispersion tended to bepresent from 4 to 8 o'clock area.

Elevation of intraocular pressure over 21 mmHg occurred in 4 of 74 eyes, 5%. Facility of outflow was decreased to less than 0.15 in 5 eyes, 7%, 6months after surgery. All these eyes manifested pathological gonioscopic features including PAS and pigment dispersion.
