積極的ステロイド点眼治療を行った実質型角膜ヘルペスの長期観察例

Longterm outcome of topical corticosteroid therapy for stromal herpetic keratitis

　実質型角膜ヘルペスに対して，角膜浮腫の短期間での消失を意図して薬力価の高い副腎皮質ステロイド剤点眼を投与し，2年以上約3年間，経過を観察した4症例を報告した。初診時の点眼投与は0.1％ベータメタゾン点眼（1日4〜6回）をIDU点眼とともに行った。全例で角膜浮腫は数週間で消失した。再発回数は1症例では約3年間に4回であったが，他の症例では2〜3年間に1〜2回であった。軽度の樹枝状上皮障害が2症例で2回および3回みられた。重篤な1例を除き最終視力はほぼ正常に保つことができた。抗ウイルス剤を併用する薬力価の高いステロイド点眼治療の有用性を確認した。

We treated 4 eyes with stromal herpetic keratitis with topical instillation of betamethasone and IDU 4 to 6 times daily. Corneal edema healed within 3weeks in all the eyes. There were instances of one to 4 recurrences during the ensuing follow-up period. Two mases canifested transient dendritic lesions. Full final visual acuity was attained in 3 eyes at the end of follow-up of 2 years or longer.
