裂孔原性硝子体出血におけるbridging vesselに対する光凝固治療

Laser photocoagulation of bridging vessels in rhegmatogenous vitreous hemorrhage

　裂孔原性硝子体出血の予防法として光凝固でbridging vesselを閉塞させる治療法を試みた。対象は，裂孔原性硝子体出血をきたした9例9眼である。Bridging vesselの，裂孔の前後約1乳頭径に対して，直接光凝固を2〜3週間隔で施行した。本法の原理は光凝固の瘢痕で色素上皮に近づいた血管に再度光凝固を行い，閉塞させるものである。2〜8回（平均5.0回）の光凝固により，全例でbridging vesselを閉塞，白線化することができた。網膜，脈絡膜出血などの術中合併症はなく，6〜24か月の経過観察中，硝子体出血の再発は認めていない。今回の治療結果から，本法はbridging vesselによる硝子体出血の再発防止に有用な方法と考えられた。

We treated 9 eyes with vitreous hemorrhage sec-ondary to retinal tear formation. We applied argon green laser photocoagulation to occlude the vessel bridging the retinal break. Photocoagulation applied directly to the bridging vessel lying in thenondeotached retinal area at 2- or 3-week inter-vals. We succeeded in occluding the vessel in all the eyes after 2 to 8, average 5, sessions. No recurrence of vitreous hemorrhage was observed during the follow-up period of 6 to 24 months. The treatment was free of complications.
