裂孔原性網膜剥離に対する硝子体手術の成績

Transvitreal approach for rhegmatogenous retinal detachment

　裂孔原性網膜剥離の復位手術として，硝子体切除気圧伸展網膜復位，輪状締結，および網膜裂孔の眼内キセノン光凝固または経強膜冷凍凝固を27眼に施行した．後極部裂孔が6眼，赤道部裂孔が15眼，経強膜裂孔閉鎖術後の再発が6眼であった．復位は26眼96％に得られた．経硝子体手術は経強膜手術では合併症の多い症例によい適応となるであろう．

Twenty-seven eyes with rhegmatogenous retinal detachment were treated by pars plana vitrectomy, hydraulic retinal reattachment with fluid-air exchange, transscleral cryoretinopexy to retinal breaks, and encircling procedure placed anterior to the retinal breaks. Reattachment resulted in 26 eyes(96%), with no recurrence nor serious compli-cations during the follow-up period ranging from 12 to 37 months, average 20 months. This method promises to be of value for cases with posterior retinal breaks and those with bullous detachment with large or multiple tears.
