糖尿病網膜症における眼底新生血管発生時の全身管理状況

Systemic factors related with fundus neovascularization in diabetic retinopathy

　糖尿病患者の眼底に新生血管を発生した時点での全身管理状況を検討することにより，糖尿病網膜症と全身的因子の関係を明らかにした。年齢的には20歳代と50歳代に二峰性のピークを示し，罹病期間は約14年であった。ヘモグロビンA₁値は10.6±2.0％，神経症は70％，の高血圧症は43％に合併した。持続的蛋白尿は33％と少なかった。新生血管発生には5年以上長期間末治療・治療中断と治療法の変更の二つの要因が深く関係し，特にそれは網膜上の新生血管よりも乳頭上の新生血管に著明であった。各々の新生血管発生に異なっ全身的因手の関与することが示唆された。

We evaluated the systemic factors related with the onset of retinal or disc neovascularization in 55 diabetic patients. The diabetes wasz insulin-depen-dent in 10 patients and insulin-nondependent in 45. The age distribution showed two peaks in the twenties and fifties. The duration of diabetes aver-aged 14 years. The HbA level averaged 10.6mg/dl. Diabetic neuropathy was present in 28 patients. Persistent proteinuria was present in 16. we identified two risk factors related with onset of fundus neovascularizatios, particularly that from the disc. One factor was absence of medical therapy for diabetes over 5 years. The other was change in the mode of therapy during the foregoing one year.
