We evaluated 73 eyes with clear corneal graft with a minimum follow-up of 10 years. We took following factors into evaluation: age of the donor, the condition that necessitated keratoplasty, frequency of episodes of graft rejection, number ofpast grafting and mean size of the donor endoth-elial cells.

There was no correlation between the longterm graft survival and donor age or frequency of rejec-tion. The nature of the host disease, number of past grafting and mean cell size were positively correlat-ed with the graft survival. The mean endothelial cell size increased gradually to 1,300μm2 for 7 postoperative years. The cell size showed wide variations among individual cases. The average cell size decreased thereafter to become stable at around 1,000μm2 after 9 years.
