角膜移植片の透明性と内皮細胞面積について

Transparency rate and endothelial cell area of the corneal graft

　角膜移植427眼について，生命表理論を応用し移植片の累積透明治癒率を算出した。その中で252眼については移植片の内皮細胞面積を経時的に測定した。原疾患が円錐角膜または水疱性角膜症であった症例に同様の検討を行った。

　累積透明治癒率は全症例では12年で65％であった。円錐角膜群では12年で90,5％，水疱性角膜症群では4年で29.0％であり，両群間に有意の差を認めた。内皮細胞面積は全症例では10年で1168μm²であった。円錐角膜群では5年で1340μm²，水疱性角膜症では3年で1840μm²であり，両群間に有意の差を認めた。水疱性角膜症における累積透明治癒率と内皮細胞面積との関係から，移植片に浮腫混濁を生ずる内皮細胞面積は2700μm²と推定された。

　角膜移植後，内皮細胞が長期にわたって変化し透明な移植片が徐々に減少することから，手術に際しては移植片に内皮細胞を多数残すことが重要であると考えられた。

In 427 penetrating corneal transplants, cumu-lative transparency was calculated using life table analysis method. Endothelial cell area was studied repeatedly in 252 corneal transplants.

Cumulative transparency rate decreased grad-ually after transplantation and was 65% 12 years after operation. In cases of keratoconus, cumulative transparency rate was 91% after 12 years and was 29% after 4 years in cases of bullous keratopathy.
