We evaluated the density of endothelial cells of 184 donor corneas and of 87 corneas after penetrat-ing keratoplasty. Specular microscopy was used throughout. Postoperative evaluations were repeated at 1, 3, 6 months and evary 6 months thereafter up to 3 years and a half. Preoperatively, there was no significant correlation between thedonor age and the endothelial cell density. Pos-toperatively, the cell density decreased with time: by 12.2% from the preoperative value at 1 month, 38% at 1 year and 57.2% at 2 years. There was no correlation between the postoperative cell loss and the donor age. We advocate that donor corneas be studied by specular microscopy before keratoplas-ty. Advanced age alone does not disqualify donated corneas.
