Abstract Purpose:To report a case of neovascular glaucoma(NVG)secondary to X-linked juvenile retinoschisis(XLRS).

Case:A 46-year-old man was diagnosed with XLRS in his childhood, with visual loss impairment and high intraocular pressure in the right eye.

Findings and Clinical Course:The best corrected visual acuity was of 0.09 in the left eye, with light perception in the right eye. The intraocular pressure was 62 mmHg in the right eye and 12 mmHg in the left eye. In the right eye, iris neovascularization was observed, and gonioscopy showed posterior synechiae. Fundus examination revealed extensive retinochoroidal atrophy centering on the posterior pole of the right eye with no retinal hemorrhage. The left eye had extensive retinal detachment beyond the arcade vessels. Fluorescein angiography showed extensive avascular areas and extra-papillary retinal neovascularization in the right eye, but no avascular area was observed in the left eye. Electroretinography showed attenuation of the b-wave, which was a negative type abnormality. Brain MRI showed no stenosis of the right internal carotid artery and ophthalmic artery. Based on these findings, the right eye was diagnosed with NVG associated with XLRS. Since the visual acuity had already reduced to light perception with no ocular pain, it was decided to follow up the patient without additional treatment. Since then, there has been no change in his condition.

Conclusion:XLRS can cause development of NVG in the longterm. Retinal detachment can cause ischemia, but an ischemic retina can result in loss of retinal detachment leading to retinochoroidal atrophy. Therefore, careful evaluation is necessary.
