Abstract Purpose:To report a case of a corneal sting by an Asian giant hornet.

Case:An 80-year-old man was stung by an Asian giant hornet in late September 2021. He visited a primary care doctor and was referred to our hospital. He had a corrected visual acuity of light perception. His cornea was cloudy, and the pupil dilation response to mydriatic eye drops was poor, preventing observation of the fundus. We washed the anterior chamber with a balanced salt solution 10 hours after the injury. We treated him with steroid eye drops and systemic administration of steroids. The corneal clouding and Descemet's membrane fold were prolonged, and we observed defects in corneal epithelium. Inflammation of the anterior chamber worsened and vitreous opacity developed. Therefore, we performed phacoemulsification and vitrectomy, and found a previously undetected tear in the anterior capsule. No abnormality in the fundus was detected at this time. The impaired corneal epithelium and endothelium and ocular hypertension were persistent four months after the injury. He was treated with eye drops. Corneal transplantation and trabeculectomy will be considered in the future.

Conclusion:The venom of the hornet is strong, and the prognosis of a hornet sting is worse than that after a bee or paper wasp sting. The prognosis may be related to the depth of the sting, injection volume of venom, and time of the initiation of treatment. In the present case, despite prompt treatment, the outcome of the injured eye was poor. In addition to early surgery, other treatments, such as intravitreal triamcinolone or steroid pulse therapy, should be considered.
