Abstract Purpose:Epithelial inclusion cysts or lymphatic cysts are well-known conjunctival cysts;on the other hand, retention cysts are rare. Here, we report a case of lacrimal gland retention cyst(dacryops)in the lateral canthus.

Case:A 48-year-old male patient presented with a mass on the conjunctiva of his right eye which cycled through periods of remission and exacerbation for 5 years. It swelled for several days, and then, shrunk with tearing. He presented to us for the treatment of the mass. At the first visit, a large mass was observed in the temporal conjunctiva adjacent to lateral canthus. However, the tumor markedly reduced on the next day, with leakage. The mass appeared to be filled with fluid with a niveau-like presentation. Surgical excision was performed. Histopathology revealed a developed lacrimal gland tissue in the subepithelial stroma. The surface of the mass was covered with normal conjunctival epithelium and there were no atypicality in lacrimal epithelial cells. No recurrence of the cysts was seen after the surgery.

Conclusion:The patient was diagnosed with a lacrimal gland retention cyst(simple dacryops)originating from the main lacrimal gland as there was no history of inflammation or surgery. The cyst seemed to shrink and expand with to opening and closing of the lacrimal duct. Dacryops may be considered as a differential diagnosis of conjunctival mass.
